- Badges will start being mailed for the 2012 Star of the North Dental Meeting beginning 3/28/2012. For more information regarding your specific badge, you can to log into your online registration account after 3/28/2012 and check to see if your badge has been printed. NOTE: You can expect to receive your badge in the mail roughly 7 - 10 business days from the date it has been printed. If you registered after April 16th you will need to pick your badge up onsite.

	- Option A: Take 94 east to the 5th St. exit. Continue past West 7th St., turn right on Washington St., Right on Kellogg Blvd. and turn left into the Saint Paul RiverCentre ramp. Handouts will be available beginning March 21, 2012.

In an effort to be more environmentally friendly, we will not print handouts for the meeting. Again this year, in advance of the meeting, we will provide course handout materials for you to access online. These materials can be downloaded and printed. We will make all handout information available for all courses for which we receive materials from the speaker. As usual, a confirmation of your registration process will be sent to you. Included with your information will be a user name and password for accessing handout materials specific to your courses. Please review your handout information in advance of the meeting to determine if you want to bring a printed copy with you.

Important 2012 Star of the North Dental Meeting Information:

Cancellation Policy: Refund requests for cancelled registration fees or ticketed programs must be received by QMS by April 6. All cancellations are subject to a $15 per person cancellation fee. If an attendee registered online, cancellations should be submitted online. If an attendee registered by mail, cancellations must be submitted in writing to QMS by April 6 at 6840 Meadowridge Court, Alpharetta, GA 30005. If tickets and name badges have been received, they must be returned to QMS before a refund will be processed. Cancelled credit card registrations will be refunded via a credit to the same card; other refunds will be made by check. Requests for refunds after the April 6 deadline, will be reviewed by the Scientific Session Committee on an individual basis at their May committee meeting. All special requests must be received before May 14 and mailed to the MDA office, attn: Refund Request at 1335 Industrial Blvd., Ste. 200, Minneapolis, MN 55413. Please include a letter explaining any extenuating circumstances and enclose unused badges/tickets.

+How do I register for the 2012 Star of the North Show?

- Attendee: Please create a username and password below and follow the steps until the registration is completed. A Pre-Registration Form may also be downloaded for sending mail by clicking here 2012 Star of the North Registration c/o QMS Services 6840 Meadowridge Court Alpharetta, GA 30005 Exhibitor:  To log into the exhibitor personnel registration system, please use the password provided in your booth confirmation e-mail.

+How do I cancel my registration/When is the cancellation deadline?

- To cancel your registration you must submit a request in writing by April 6, 2012. You can submit your request by sending an e-mail to mda@prereg.net, sending a fax to (678) 341-3099, or mailing a cancellation letter to: 2012 Star of the North Registration c/o QMS Services 6840 Meadowridge Court Alpharetta, GA 30005 APRIL 6, 2012 CANCELLATION DEADLINE - Cancellation requests must be received BY QMS by the end of the day in order to be honored. Badges/tickets must be returned in order for a refund to be issued. Refunds will be processed after the Star of the North meeting. Cancellation Fee: $15 per cancelled paid registration (i.e., per paid person, not per paid course)

+How do I make changes/additions to my registration?

- To make changes/additions to your registration please log back into your registration. Once you are logged in you will see the options to Add attendee, Edit Contact info, and Add Events . Follow the steps to complete the change. You can also contact QMS at (678) 341-3063 to make changes over the phone.

+How do I get a new confirmation/receipt?

- Please log back into your registration and click on the resend confirmation button. If you have a balance on your account you will be directed to the payment screen. You can also contact QMS at (678) 341-3063 or email mda@prereg.net to request a receipt.

+When will I receive my badge?

- Badges will be mailed beginning March 28th. Please log in to your account to check and see if your badge has been printed. Please allow 7 to 10 business days for your badge to arrive. If you registered after April 16th you will need to pick your badge up onsite.

+Is there a charge for my spouse?

- The charge for the spouse of a dentist (non-dentist) is $25. Spouses are not eligible to receive Continuing Education Credits.

+How do I get my online handouts?

- Handouts will be available online beginning March 21, 2012. Please go to http://star.mndental.org/ and click on the Handouts Link to download. You will need your badge/account number and last name. If you do not have your badge/account number please call (678) 341-3063 for assistance or email mda@prereg.net.

+How do I redeem my CE credit?

- At the end of each class you will be given a four digit code to verify your attendance. Once you have the code you can go to http://star.mndental.org/ and click on the CE Verification Link to enter your codes and print your certificate. CE Verification kiosks will also be available onsite in the exhibit hall to redeem credit.

+What if I can't stay for the entire course?

- Completion codes are only given at the conclusion of the course. Full credit should not be expected for courses that are not attended in their entirety, so do not arrive late or leave early.
