REFUND POLICY
Refund requests for ticketed programs or paid registrations must be made in writing to QMS by April 5, 2010. All tickets and name badges must be returned to QMS before a refund will be processed. Cancelled credit card registrations will be refunded via a credit to the same card; other refunds will be made by check. Cancelled paid registration will be subject to a $15 per person cancellation fee. Refunds will be processed AFTER the meeting, so please allow up to six weeks after the meeting for refunds to be received.

Requests for refunds after the April 5 deadline, will be reviewed by the Scientific Session Committee on an individual basis at their May committee meeting. All special requests must be received before May 10 and mailed to the MDA office, attn: Refund Request. Please include a letter explaining any extenuating circumstances and enclose unused badges/tickets.
